Midwives
Each Children’s Centre has a named midwife contact (see attached list) There is a project underway in central Bristol whereby midwives are completing the Children’s Centre Registration Form with pregnant women at their booking appointment in the early stages of pregnancy. Children’s Centres need to ensure that they offer appropriate services to women who are at least 24 weeks pregnant.
Midwives and health visitors use the ‘Request for Service Form‘ in order to refer any family to a Children’s Centre and Children’s Centres must respond to the referrer using the last section of the form to confirm what services have been offered to the family. Download the form here.
