Health Visitors
A partnership agreement is in place between health visitors and Children’s Centres. Each Children’s Centre has a link health visitor and the health visiting team use the ‘ Request for Service Form’ in order to refer a family to a Children’s Centre. An information sharing protocol is in place which enables information about new babies born in Bristol and children under 5 moving into or within Bristol to be shared with each Children’s Centre on a monthly basis.
